Dear Dr. Singh:

Enclosed are two sets of forms to be completed by you, my treating psychiatrist, in connection with the Social Security Administration's review of my disability claim.

The D.C. Rehabilitation Services Administration requires that you forward the completed forms within 10 days (June 16, 1997) to the agency. Enclosed are two return envelopes.

Enclosed are materials that may assist you in the completion of the forms. Please do not forward these informational documents to the agency; I have already provided the following documents to the Social Security Administration.

1. Results of psychological testing performed by the George Washington University Medical Center (GW) Ramin Mojtabai, M.D., dated March 1996.

2. Letter dated February 14, 1996 from Dimitrios Georgopoulos, M.D., my treating psychiatrist at GW, stating the diagnosis paranoid schizophrenia (295.30).

3. Letter dated January 22, 1996 to Dr. Georgopoulos detailing the nature of my beliefs. I continue to believe that all of the assertions in the letter are accurate representations of reality. I continue to believe that I am under continuous surveillance by attorney managers at the law firm of Akin, Gump, Strauss, Hauer & Feld, located in Washington, D.C.
