Dear Dr. Singh:

The U.S. Social Security Administration has advised me by letter dated April 26, 1997, a copy of which is enclosed, that the agency will be conducting a review of my eligibility for disability benefits.

This will confirm the following facts concerning my psychiatric treatment at the P Street Clinic.

1. I was evaluated for treatment by the District of Columbia Commission on Mental Health services commencing with an initial consultation, on June 6, 1996, with Marjorie Heydt, MSW. I had several evaluation consultations with Mrs. Heydt during the period June-August 1996.

2. I underwent a standard psychiatric evaluation conducted by Dr. Ghaddai at the P Street Clinic on August 29, 1996. I discussed with Dr. Ghaddai my belief that I had been under surveillance by attorney managers of my former employer, the law firm of Akin, Gump, Strauss, Hauer & Feld, since late October 1988. Dr. Ghaddai appeared to conclude that my belief was delusional. I experienced an eruption of intense anger during my consultation with Dr. Ghaddai, and Dr. Ghaddai stated his belief that there was a relationship between the anger I directed at him and my employment-related interpersonal difficulties.

3. In August 1996 Mrs. Heydt advised me that, in consultation with attending physician Stephen D. Quint, M.D., it had been decided that I be assigned to Mrs. Heydt for psychotherapy.

4. At my initial therapy session with Mrs. Heydt on August 29, 1996 I experienced an eruption of intense anger directed at Mrs. Heydt. Mrs. Heydt directed me to leave her office, and advised me that, in view of the apparent severity of my illness--as evidenced by my intense anger--she could not continue to treat me. Mrs. Heydt advised me that she would consult with Dr. Quint, her supervisor, to make alternative arrangements for my treatment.

5. I consulted with Mrs. Heydt and Dr. Quint on October 30, 1996. The content of that consultation is summarized in a letter to Dr. Quint, dated November 2, 1996, a copy of which is enclosed for your reference. On the basis of the consultation with Dr. Quint on October 30, 1996 I was assigned to Dr. Singh, a physician-psychiatrist, for psychiatric treatment.

6. I have had consultations with Dr. Singh, one time per month, beginning in October 1996, which consultations continue to the present time. The frequency of the sessions (one time per month) is for reasons of administrative convenience (staffing limitations at the clinic) and does not reflect my needs or the severity of my illness. Dr. Singh has advised me that he would like to see me more frequently, and that arrangements for more frequent psychiatric consultations may become feasible in the future.
