Should I File A Complaint?

Mr. Freedman:

The Office of Health Care Ombudsman and Bill of Rights has received your complaint concerning Medicare Fraud by Nurse Sara F. Carroll.

We need additional information to proceed with the filing of a complaint.  This information should include a description and dates of the alleged fraud committed by Nurse Carroll.

Subject: Medicare Fraud by Nurse Sara F. Carroll

To the D.C. Department of Health:

I am prescribed medication by nurse Sara F. Carroll at the McClendon Center, a DC Core Service Agency for mental illness.  I consult nurse Carroll once every two or three months for medical management for unspecified mental illness.  Nurse Carroll continues to treat me and bill Medicare and Medicaid (QMB Program) for her services despite the fact that I do not now nor have I ever suffered from mental illness.  I believe nurse Carroll's behavior constitutes a fraud on the Medicare system.
