Dear Dr. Pitts:

This will advise that I filed for Social Security Disability Benefits on April 20, 1993, in connection with which I have executed a release, on file with the U.S. Social Security Administration, authorizing your release to the agency of all mental health information in your possession concerning my case.

A representative of the Social Security Administration will be contacting you in the near future regarding my case. You are authorized to release all mental health information, including copies of all correspondence and other writings and tape recordings that I submitted to you.

I had provided to Dr. Pitts tape recordings of several telephone conversations I had with my sister.

The conversations, in my opinion, did not evidence symptoms of mania described in my initial assessment performed by Dr. Cuenco: the mood congruent psychotic features of loose associations; flight of ideas; and pressured, rapid speech.

I am not responsible for what evidence the Social Security Administration chose to review or not review.
