GW Psychiatric Treatment: Receipt of Inital Assessment

Pursuant to the following request, my then treating psychiatrist at GW, Suzanne M. Pitts, M.D. provided me a copy of Dr. Cuenco's inital assessment chart.  The Social Security Administration had approved my claim for disability benefits effective August 17, 1993, about two weeks before.   If SSA had obtained a copy of the initial assessment chart at the time it was considering approval of my claim (April 20, 1993-August 17, 1993), it was not via me.

Dear Dr. Pitts:

This letter will serve as formal written notice, per the client access provision of The District of Columbia Mental Health Information Act, D.C. Code section 6-2041, that I wish to duplicate the record of mental health information concerning the following:

1.  Evaluation (initial assessment) consultation conducted by Dr. Napoleon Cuenco on September 1, 1992.

2.  Evaluation (initial assessment) consultation conducted by Dr. Napoleon Cuenco on September 8, 1992.

3.  Written documentation relating to departmental conference concerning patient assessment, held in about late September 1992.

  1. I learned Dr. Cuenco's diagnosis of bipolar disorder in September 1992, when GW sent me a bill for the initial assessment.
