Letter to Social Security Administration re: Misdiagnosis by GW

RE:  Social Security Disability Claim No. xxx-xx-xxxx

Dear Sir:

The U.S. Social Security Administration approved my claim for disability benefits in August 1993, determining that the effective date of disability was October 29, 1991.

In September 1992 I underwent a two-hour assessment at The George Washington University Medical Center Department of Psychiatry and Behavioral Science (GW) conducted by Napoleon Cuenco, M.D. under the supervision of attending psychiatrist, Daniel Tsao, M.D.   Attached is a copy of the Initial Assessmentdated September 24, 1992.  Dr. Cuenco diagnosed bipolar disorder (rule out schizoaffective disorder).  In February 1993 Suzanne M. Pitts, M.D. (deceased) at GW prescribed lithium for bipolar disorder.  I do not now nor have I ever suffered from bipolar disorder or schizoaffective disorder.  See enclosed email message dated September 17, 2013 from Dennis Hobb to Gary Freedman listing my current diagnoses as Delusional Disorder, Major Depression Recurrent Severe Without Psychotic Features, Alcohol Dependence in Sustained Remission, PTSD, and Schizoid Personality Disorder.

Dr. Cuenco noted in the Initial Assessment dated September 24, 1992 that my intelligence was average and that my insight was poor.  Psychological testingperformed in May 1994 by GW disclosed that my overall IQ was 125 (95th percentile) and that my verbal IQ was 136 (99th percentile).

I enclose a lengthy document that comprisesmy own view of my personality problems; the document serves as a comprehensive self-created personality profile.

I currently receive medical management provided by The McClendon Center, a D.C. Core Services Agency (Nurse Sara F. Carroll).  I also receive weekly psychotherapy provided by the D.C. Department of Behavioral Health (Mohammed Shreiba, M.D. under the supervision of Earle Baughman, M.D., St. Elizabeths Hospital, Washington, DC).
